Presenter's Checklist for Submission

Here you will find required forms and relevant information for those accepted to present (symposia workshops, paper and poster presentations) at the 23rd Annual Meeting and Symposium in Aventura, Florida on December 6-9, 2012.  All presenters must also register for the 23rd Annual Meeting and Symposium.  Please click on the topics below for more information.

Please note:

  • All speakers MUST submit slides for inclusion in participant materials.
  • Faculty disclosure forms MUST be submitted for all symposia presenters.
  • Presenter biosketches must be submitted for all symposia presenters.  These biosketches should adhere to a 1-2 paragraph format in a Word document.  (These are NOT the NIH biosketch forms.)
  • Principal authors will be notified of review decisions by March 1, 2012.
